Shalom Free Health Clinic - 2014 Amount Funded: $3,000.00
In 2011 they provided approximately 80 Stoughton residents with free medical services in 510 visits. This included prescription medications, medical examinations, mental health care, nutrition counseling, physical therapy, social work assistance, spiritual consultation, and education.

Stoughtons Shalom Free Health Clinic - 2016 Amount Funded: $4,500.00
The purpose of Stoughtons Shalom Free Health Clinic is to make health services accessible and available to low income residents in the Stoughton and outreach areas who have no health insurance or inadequate of health insurance. Having regular providers enhances monitoring of health and allows a distinct advantage with ongoing education, and subsequent health enhancement of lifestyle. In addition, Stoughton Hospital benefits through reduced utilization of the ER which results in cost savings. Referral agencies benefit by having a professional and credible resource to serve clients. Stoughton community benefits as those of low income gain health, become educated, and make lifestyle changes.
Stoughtons Shalom Free Health Clinic operates one day per week from 5:00 pm to 9:00 pm. It has a staff of 15 medical, nursing, mental health, clergy, and social service workers. It also has a limited part time clinical coordinator. Patients are seen by appointment and provided the following health care: medical examination and prescribed treatment as needed, mental health assessments provided both by a psychiatrist and counselor, spiritual consultation for the purpose of addressing spirituality and the state of being, social work assistance, nutritional counseling as needed, patient education on matters of health and wholeness, and as necessary, referral of patients to a specialty free clinic in Madison.

Shalom Free Health Clinic - 2015 Amount Funded: $4,000.00
In 2011 they provided approximately 80 Stoughton residents with free medical services in 510 visits. This included prescription medications, medical examinations, mental health care, nutrition counseling, physical therapy, social work assistance, spiritual consultation, and education.
The Shalom Free Health Clinic (project) operates one day per week from 5:00pm through 9:00pm. It has a volunteer staff of 18 medical, nursing, mental health, clergy, social service workers, and office assistants. It has a limited part time clinic coordinator. Patients are seen by appointment and are provided health care in accordance to a holistic approach which includes the following: medical examination and prescribed treatment as it is needed; mental health assessment provided by both a psychiatrist and counselor with corresponding intervention as needed; spiritual consultation for the purpose of addressing spirituality and the state of being; social work assistance for addressing life circumstances which may affect mental health; nutritional counseling as it is needed; patient education on matters of health and wholeness; referral of patient to a specialty free clinic in Middleton.

Affordable Transportation Program - 2016 Amount Funded: $2,500.00
It is the desire of Stoughton United Ministries to provide services that will fill existing social service gaps in the local setting and enhance the level of assistance provided to the poverty level in-need in the Stoughton School District. In that light, the Affordable Transportation Program offers needed transportation services from Stoughton to Madison and back for the low income in need as other such services do not currently exist. Transportation services offered help to assure medical appointments, court appointments, business appointments, skills training, interview opportunities, and other self-betterment opportunities.
The Affordable Transportation Program involves a paid coordinator (.1 FTE) and six volunteer drivers who utilize their personal vehicles to transport program participants. Volunteer drivers are reimbursed for their mileage. The goal of the program has been to provide accident free round trips to eligible persons for the purpose of medical appointments, court appointments, job interviews, skills training, and other personal betterment reasons as well as to provide transportation to local food pantries.
